The Occupational Therapist evaluates residents to develop and implement individualized treatment plans aimed at maximizing independence and quality of life. They are also responsible for supervising COTAs and collaborating with an interdisciplinary team to coordinate resident care and discharge planning.
Candidates must be graduates of an accredited Occupational Therapy program and hold a current state license in good standing. Experience in skilled nursing or post-acute care settings is preferred.
City View Post Acute is seeking a compassionate and skilled Occupational Therapist (OTR) to join our rehabilitation team on a PRN basis. The Occupational Therapist evaluates and treats residents, develops individualized treatment plans, communicates with families and interdisciplinary team members, and maintains accurate documentation in accordance with regulatory standards. Evaluate residents to determine occupational therapy needs, functional limitations, and rehabilitation potential in accordance with physician orders and facility policies. Develop, implement, and modify individualized treatment plans designed to improve residents' functional independence and quality of life. Assist the nursing department with training and support of Restorative Aides. Supervise Occupational Therapy Assistants (COTAs) in direct resident care and related activities in accordance with state practice regulations. Communicate regularly with physicians, nursing staff, interdisciplinary team members, residents, and families regarding resident progress, goals, and discharge planning. Participate in Resident Care Conferences, Utilization Review meetings, Rehabilitation meetings, and other interdisciplinary team meetings as needed. Provide in-service education and training programs for facility staff. Complete timely, accurate, and compliant documentation in accordance with facility policies, governmental regulations, and third-party payer requirements. Maintain a safe treatment environment by adhering to infection control procedures, safety standards, and equipment maintenance protocols. Educate residents, families, and caregivers regarding treatment goals, home exercise programs, safety techniques, maintenance programs, and fall prevention strategies. Recommend and facilitate the procurement of durable medical equipment and adaptive devices to support resident independence with activities of daily living (ADLs), including splints, assistive devices, grab bars, elevated toilet seats, and other adaptive equipment. Maintain strict confidentiality of resident and company information. Report equipment issues promptly to ensure safe and effective operation. Contribute to quality assurance initiatives, performance improvement programs, and rehabilitation department development. Foster positive and professional relationships with residents, families, staff members, and healthcare professionals. Supervisory Responsibilities Supervise Certified Occupational Therapy Assistants (COTAs), rehabilitation aides, and occupational therapy students as assigned. Qualifications Graduate of an accredited Occupational Therapy program. Current Occupational Therapist license in good standing within the state of practice. Strong clinical assessment, treatment planning, and documentation skills. Knowledge of applicable federal, state, and local regulations governing therapy services. Excellent communication, interpersonal, and organizational skills. Experience in skilled nursing, post-acute care, or rehabilitation settings preferred. Why Join City View Post Acute?
