The Primary Care Physician leads clinical decision-making and develops personalized care plans for PACE participants within an interdisciplinary team. Responsibilities include performing comprehensive health assessments and managing chronic conditions to help seniors live independently.
Candidates must hold an MD or DO degree, be Board Certified in Internal Medicine or Family Medicine, and possess an active Michigan medical license. CPR certification is required, while experience in geriatrics or palliative care is preferred.
