OVERVIEW: This position is responsible for assisting the Registered Nurse within the realm of their education with respect to any changes with the patient, as well as desk duties, and answering the phones.
Essential
Functions
OF
JOB: (other duties may be assigned) Monitor vital signs such as blood pressure, temperature, pulse and respiration. Obtain vital sign readings four or more times per 24 hours. Document vital readings at time of observation and report any changes to the Registered Nurse. Provide patient care. Bathe, clean incontinent patients, and provide mouth and peri-care at least once daily and more often as necessary. Report and note any skin changes or open areas or any other changes. Observe and report any changes in respiratory or mental status. Perform accu-checks and measure patient intake/output of fluids. Perform accu-checks four times per day and/as ordered by the physician. Record and document results of accu-checks and report changes to the Registered Nurse immediately. Document electronically all PO intake as well as output to include urine, stool and NG drainage. Document intake/output results at the end of each shift. Provide feeding assistance. Feed patient three or more times per day. Record for each meal period how the patients ate and record the food intake three times a day at breakfast, lunch, and dinner. Ensure patient safety and respond to emergency codes as needed. Continually ensure the patient room is safe, hallways are safe and an overall safe environment is provided. Transfer, ambulate and transport patients in a safe manner. Respond to all blue, red, black, pink, winhos, and utility failure codes within five minutes. Ensure staff follows appropriate Hospital policies and procedures. Apply telemetry. Place the telemetry unit on the patient and make sure it is transitioning properly. Put patient’s name, physician and room number into the monitor. Perform enemas. Perform the enema while ensuring the patient’s comfort and safety. Document results and notify Registered Nurse. Complete general housekeeping duties. Remove dirty or soiled linens and replace with clean linens with the patient in or out of bed at least once per day. Ensure the patient’s comfort and safety. Clean IV pumps and keep equipment clean and functioning properly. Put away clean linens daily. Ensure there are enough supplies for the next shift. Order supplies, answer telephones, and assist with improving unit clerk practices. Assist with supply orders on a weekly basis. Monitor accu-check supplies. Ensure that telephone calls are answered as promptly as possible within three rings. Use proper telephone etiquette when answering the telephone by identifying self, position, etc. Maintain 100% compliance with all Hospital policies and procedures. Participate in quality improvement, fires safety, infection control, etc.

JOB: Provide post-mortem care. Admit patients. Apply cold and warm compresses. Change simple dressings. Respond to code red and code black. Restrain patients. Empty Foley bags and assist patients with using the restroom. Perform other related duties as assigned or required. Follow standard precaution guidelines.

QualificationsQUALIFICATIONS:
Education/Training/License/Certification: High school diploma or equivalent required. Completion of PCT program or comparable program such as (EMT, Medical Assistant, Nursing student with at least one semester of clinical), preferred. BLS Healthcare card is preferred at time of hire, required within 6 months of hire.
