Assess the psychosocial needs of patients and their environments, sharing findings with the care team.
Conduct social evaluations and create intervention plans based on findings.
Maintain detailed clinical records for all referred patients.
Prepare comprehensive clinical and progress notes.
Provide patients and families with resources and referrals to address practical and environmental needs.
Deliver care with passion and compassion in every interaction.

Education: Graduate of a master’s program in social work accredited by the Council on Social Work Education.
Licensure: Current licensure in the state of Michigan. - Cannot accept Limited License MSW
Experience: At least one (1) year of Home Health experience required.
