Responsibilities

• The social worker will conduct assessment to determine patient needs and provide support by developing and implementing individualized treatment.
• The social worker will provide counseling and support to patients and their families, focusing on accessing additional resources and addressing mental health needs.
• The social worker with monitor and evaluate patient progress and adjust treatment plans as necessary.
• They will collaborate with multidisciplinary team to provide comprehensive and passionate care.
• The provider will serve as a role model for Mercy’s mission and values-is caring, ethical, fair, respectable, compassionate, courteous and hospitable.

Requirements

Minimum of LSWA, LGSW or LCSW. LCSW-C preferred Preferred skill set includes

Accurate verbal and written expression Work organization, problem solving Psycho-social counseling Conflict resolution Identification of crisis situations Experience with Bio-psychosocial assessments Experience with Crisis counseling Experience with Community resource referrals Experience with Documentation Must possess a Bachelors or Masters Degree is Social Work Fluency in Spanish preferred
