About the role

Provide primary care to patients in long-term care facilities, including developing personalized treatment plans and managing medications. Collaborate with multidisciplinary teams and maintain accurate medical documentation while educating patients and families.

As a Primary Care Provider, you will provide primary care to patients in long-term care facilities.

This includes

  • Developing and implementing personalized treatment plans based on patient needs
  • Offering medication management, including prescribing and monitoring medications
  • Collaborating with a multidisciplinary team of healthcare professionals to ensure patient care
  • Maintaining accurate medical documentation in line with regulatory standards
  • Providing education and support to patients and their families regarding mental health conditions and treatments

Qualifications

  • Current Nurse Practitioner license in specified location
  • At least two years of experience is required
  • Experience providing care to geriatric populations in long-term care settings is preferred but not required
  • Excellent communication skills, especially when working with facility staff and residents' families
  • Ability to manage time effectively, including flexibility in scheduling and being responsive to patient needs

Requirements

Requires a current Nurse Practitioner license and at least two years of experience. Experience with geriatric populations in long-term care settings is preferred.
