We are seeking a compassionate and dynamic individual to join our team as a Care Coordinator for the unit. In this role, you will have the opportunity to coordinate care for our patients, making a meaningful difference in their lives and supporting them on their journey to recovery. - DUTIES
- AND
- RESPONSIBILITIES: The following tasks are necessary for the role. Additional duties may be assigned as needed. Responsible for the development of care planning and patient needs assessments. Facilitates Life Skills groups following Crossroads Turning Points' policies and procedures. Acts as liaison, establishing networks and collaboration with hospitals, community health centers, and state and human service agencies about patient services and needs. Document admissions, care management needs, discharge, and discharge planning. Administer case management assessments to identify patient needs and ensure the Addiction Severity Index (ASI) is completed at intake. Organize a schedule of patient bed dates and transition levels of care and maintain consistent communication with Medicaid Placement Coordinators. Assists treatment team with patient intake, transfer, and discharge assessments. Collaborating with facility treatment teams (medical, psychiatric, and SUD) to identify patients' needs. Assist patients in meeting their basic needs (e.g., clothing, medical, judicial, religious) from available resources.

- QualificationsQUALIFICATIONS:
- Education: An associate or bachelor’s degree in a behavioral health-related field is preferred, or a related field. Obtain the Certificate of Addiction Technician (CAT) certification within 18 months of the hire date. Obtain case management certification within 90 days of hire.
Experience
Minimum of two years of experience working in the substance use and mental health field.
Other
- Must have a good understanding of substance use, mental health, and treatment delivery systems. Advanced computer skills required. CPR and first-aid certification. (The agency pays for initial training and renewals. Must be able to take vital signs, recognize possible medical problems, and refer to the medical staff when necessary. Valid driver's license and good driving record (must provide a current, unofficial DMV driving record upon hire). Able to communicate effectively in a multidimensional treatment program, including medical, psychiatric, and substance misuse-related.
