The Case Manager advocates for clients to ensure access to medical, social, and educational services while coordinating care among multiple service providers. They are responsible for developing person-centered recovery plans and maintaining detailed documentation to meet medical necessity standards.
A Bachelor's degree in psychology, social work, or a behavioral science field is required, with specific experience requirements for SMI, SED, or substance use populations. Candidates must possess a valid driver's license and obtain a Targeted Case Management certification within six months of hire.
