Provide skilled in-home physical therapy services, including evaluations and the implementation of individualized treatment plans. Educate patients and caregivers on fall prevention and maintain compliant Medicare documentation.
Must hold a current Hawaii license as a PT or PTA with at least one year of clinical experience. A valid driver's license and reliable transportation are required for field work.
