Coordinates medical care for high-risk patients through risk assessments and targeted outreach to close gaps in care. Develops and implements care plans for medically complex patients while managing clinical and ancillary resources.
Requires a Bachelor's degree in Nursing or a health-related field and 3 years of professional nursing experience. Must hold a valid NYS RN license and current CPR certification.
