The Case Manager Aide performs community outreach and conducts initial screenings to assess client eligibility for services. They are responsible for gathering documentation, providing HIV testing, and offering PrEP education and referrals.
A high school diploma or GED is required, while an associate's or bachelor's degree in health or human services is preferred. Preference is given to candidates with at least 2 years of experience working with HIV-risk populations and a background in HIV testing.
Recruitment/Assessment/Care Coordination CMA will perform outreach/recruiting within the community, informing perspective clients of CM services and scheduling assessments for eligibility. CMA will schedule and complete an Initial Screening appointment using a short screener to assess eligibility for CM services and educate on PrEP. CMA will gather eligibility documents and upload/update them in eCW as needed. Referrals will be made for CM Intake appointments, other FMCS services or other community organizations to address client needs. HIV testing and Education will be provided to status neutral clients. Provide PrEP education and referrals. Office Hours: Monday - Friday, 8:30am – 5:30pm
Associate’s/bachelor’s degree in health or human services related field preferred. High School diploma or GED required. A minimum of 2 years of experience working with persons living with HIV or at high risk of HIV acquisition preferred. HIV Testing background preferred.
