Provide pharmaceutical care by developing evidence-based patient care plans and managing medications for chronic diseases. Coordinate with healthcare teams to prioritize patient flow and perform medication reconciliation for transitions of care.
Requires an RPh or PharmD degree and valid licensure in Washington and/or Idaho. A minimum of one year of clinic experience or residency is preferred, along with BLS certification and a valid driver's license.
Purpose of Job: Improve the overall health of the communities we serve by providing pharmaceutical care in the following ways: Essential
Provides drug education and counseling to patients as outlined in department policies and procedures Develops, documents, and executes a rational, safe, evidence-based, cost-effective patient-centered plan of care with patient’s PCP and members of the care team utilizing national guidelines or established protocol. Identifies patient strengths and barriers to pharmacotherapy plan and medication adherence. Reviews, authorizes, and changes medications for specified chronic diseases within the delegated prescriptive authority and guidelines under CHAS Health’s state approved Collaborative Drug Therapy Agreement: Identify and take specific corrective action for drug-induced problems Monitor patients for appropriateness and response to therapy Administer injectable medications and immunizations Order and analyze laboratory and diagnostic test data so as to modify drug therapy and dosing as necessary Coordinates with providers, registered nurses, medical assistants and pharmacy staff to prioritize patient care and provide efficient patient flow in the clinic. Utilizes registry management tools. Notifies primary provider of pertinent/atypical findings through documentation in the electronic health record (EHR) Provides effective transition of care needs including, but not limited to, medication reconciliation and completing post-discharge phone calls Identifies opportunities for the development of new care management approaches; takes an active role in the development and implementation of quality improvement projects Performs other duties assigned, including supporting the CHAS Health Mission and Core Values.
Education/Experience: RPh or PharmD from accredited school of pharmacy. Current valid licensure in the state of Washington and/or Idaho required. Minimum of 1 year experience in a clinic setting as a clinical pharmacist or residency experience preferred. Prior non-profit experience preferred. Ambulatory Care experience, Board Certification and/or Certified Diabetes Educator (CDE) preferred. Experience with process improvement and supervisory experience a plus.
