Provide care coordination through the Healthy Start Program for eligible pregnant women to ensure access to prenatal and post-partum care. Connect families to social services and provide education on maternal and baby care to achieve healthy birth outcomes.
Requires a high school diploma or equivalent and a valid Ohio driver's license with automobile insurance. Knowledge of community resources is required, and a Certified Community Health Worker credential is preferred.
