The nurse will assess patient needs to develop and implement individualized care plans for end-of-life journey. Responsibilities include coordinating with interdisciplinary teams, monitoring symptoms, and providing emotional support to patients and families.
Candidates must hold a valid Arizona RN license and have at least two years of nursing experience, preferably in hospice or palliative care. Strong assessment skills and knowledge of hospice regulations are required.
Assess patients' health care needs and develop individualized care plans. Coordinate patient care with physicians, family members, and other healthcare professionals. Ensure compliance with state and federal regulatory requirements in hospice care. Monitor patients' symptoms, medication responses, and overall progress. Provide education and emotional support to patients and their families. Facilitate and attend interdisciplinary team meetings to discuss patient care. Document patient care accurately and in a timely manner in accordance with company policies. Valid AZ RN license in the state of practice. Minimum of two years of nursing experience, preferably in hospice or palliative care. Strong assessment and interpersonal skills to effectively communicate with patients and families. Ability to work independently and as part of a collaborative team. Compassionate and empathetic approach towards patient care. Knowledge of hospice regulations and best practices. Basic computer skills for documentation and communication purposes.
