This is a great opportunity to build meaningful connections, deliver personalized care, and see the impact of your work every single day-all in a supportive home setting. 🩺 What You'll Do: Provide physical therapy treatment plans as established by the supervising Physical Therapist Provide patient education and instruction in home exercise programs Monitor, document, and report patient progress and any changes in condition Support patient in achieving functional goals that enhance safety and independence Collaborate with a multidisciplinary team to ensure continuity of care Document visits and patient progress accurately and timely ✅ What You'll Need: Current Physical Therapist Assistant (PTA) license in the state of Pennsylvania.
