This LPN position will be held in a Day Program setting. The LPN will cross train at Gardner location. The Licensed Practical Nurse (LPN) reports to the Program Director for all programmatic duties, and is supported by the Assistant Director of Nursing for all nursing-related duties, policy, and practice. This position will be compensated on an hourly basis.
Essential duties and responsibilities
Oversee all health-care services provided to the individuals at assigned programs
Participate in the continuing interdisciplinary evaluation and review of each individual’s needs both in scheduled Interdisciplinary team meetings and ongoing at the programs
Obtain reports and approval of treatment plans as required by regulated timelines
Uphold individuals’ fundamental rights and dignity by ensuring respectful, person-centered supports
Maintain individuals’ files and document any recommendation resulting from the periodic reviews
Management of medication as per CMR 419.431
Conduct medication reviews a minimum of every three (3) months, or more frequently as needed Monitor and record vital signs and weights every six (6) months, or on an as needed basis
Contact family/community residences, physicians, other health care professionals and service coordinators as necessary regarding specific medical issues

Screen admission applications for medical information prior to acceptance and request missing documentation in collaboration with Day Hab Directors
Monitor the general health and well-being of individuals daily and communicate any concerns or issues to Program Director and Assistant Director of Nursing
Obtain and maintain medical records including yearly update of emergency fact sheet, annual physicals, and authorization for emergency medical care
Develop and maintain nursing care plans
Document nursing progress notes as daily, quarterly, semi-annually and annually
Adhere to all medical regulations and requirements for federal and state governing agencies, funding and accreditation sources, and Venture policies and procedures
Provide ongoing training to individuals regarding self-care, hygiene and other relevant topics
Oversee completion of maintenance-therapy treatment as recommended by OT/PT/SLP in conjunction with program management team
Attend and participate in meetings, trainings and development as required
Actively collaborate with peers to problem solve and coordinate best practices for high acuity individuals
Maintain confidentiality of all individual information, records, and sensitive communications in accordance with agency policy, HIPAA, and all applicable regulations.
Maintain CPR, AED, and other required certifications
Other duties as assigned.

Requirements
Current and valid registration as a professional nurse under the Massachusetts Board of Registration in Nursing.
At least one year of full-time working experience as an LPN.
