Requires a high school diploma or equivalent and a valid driver's license with a vehicle. Candidates must have at least one year of experience working with individuals with disabilities.

To promote the mission of PATH to help individuals achieve a more independent and fulfilling life by being an effective team member. Responsible for the training and support of individuals in the program.
1. Assist in the coordination of training and activity planning for individuals in the program.
2. Assist with the preparation for annual DPW Licensing standards and Office of Long-Term Living Licensing.
3. Meet productivity standards of the ADP Services.
4. Assist in the development of assessments for assigned individuals.
5. Attend, as needed, individual team meetings, ISP meetings, QEST reviews, SIS meetings, Vision for Equality monitoring and Supports Coordination monitoring.
6. Explore and facilitate community integration opportunities for individuals.
7. Meet regularly for supervision with your supervisor.
8. In conjunction with Human Resources, participate in related investigations.
9. Assist individuals to and from the van each day.
10. Provide assistance and supervision of individuals with personal care during group and individualized activities.
11. Drive the van for programmatic reasons (during program hours) on an as-needed basis.
12. Complete assigned responsibilities related to the upkeep of the facility.
13. Attend staff meetings
1. HS Diploma or equivalent.
2. Valid driver's license and vehicle required.
3. At least one year of experience with individuals with disabilities.
