The Care Coordinator manages individualized service coordination, including transportation and doctor visits, for all clients. They are responsible for monitoring client safety, maintaining clinical documentation, and facilitating therapy sessions.
Candidates must hold a Master's Degree in a relevant field such as Psychology or Social Work and be licensed or eligible for licensure. A minimum of one year of experience in mental health and valid CPR/First-Aid certifications are required.
Under the supervision of the Program Director the Care Coordinator is responsible for anything related to the coordination of care for all ROWI clients. The Care Coordinator will coordinate individualized service including orientation, transportation, doctor visits, and schedule.
Educational Background: Minimum of a Master’s Degree in Marriage and Family Therapy, Counseling, Psychology, or Social Work from an accredited institution.
Must be licensed or eligible as an MFT, Clinical Psychologist, or Social Worker. Unlicensed clinicians will receive supervision.
