• This position will include field visits in Queens and Nassau counties, in addition to in-patient visits.

Position Overview Reporting to the Hospice Administrator, the Hospice Social Worker delivers appropriate social work services to assigned patients, residents, family members and significant others. The Social Worker is responsible for performing initial psychosocial, emotional, spiritual and bereavement assessments, ongoing assessments, and assists in the development and implementation of goal-directed IDG care plan while serving as an advocate for the patient and family members.

Position Qualifications Master's Degree in Social Work Active NYS Social Work License Prior experience as a Social Worker in a community health, hospice or inpatient setting Valid New York State driver's license and able to make field visits in Queens and Nassau counties
