The Hospice Social Worker provides psychosocial, emotional, and spiritual assessments for patients and their families. They develop and implement goal-directed care plans while serving as a patient advocate within an interdisciplinary team.
Candidates must hold a Master's Degree in Social Work and an active New York State Social Work License. A valid NYS driver's license is required for field visits, along with prior experience in hospice, community health, or inpatient settings.
Position Overview Reporting to the Hospice Administrator, the Hospice Social Worker delivers appropriate social work services to assigned patients, residents, family members and significant others. The Social Worker is responsible for performing initial psychosocial, emotional, spiritual and bereavement assessments, ongoing assessments, and assists in the development and implementation of goal-directed IDG care plan while serving as an advocate for the patient and family members.
