This role involves providing intensive, community-based support and advocacy to help persons navigate mental health, legal, and social challenges. You'll work as part of a multidisciplinary team, delivering services directly in the home and community. Ideal for compassionate professionals who thrive in a fast-paced, mission-driven environment.
Program for Assertive Community Treatment (PACT) provides comprehensive treatment to people with serious and persistent mental illness. People served by PACT receive individualized multidisciplinary services that include staffing available 24 hours a day/ 7 days a week, and a staff Psychiatrist.

Serve as a dedicated job coach, offering consistent, on-the-ground support to participants while fostering strong, collaborative relationships with employers.
