Responsible for providing service coordination activities and assessments for an assigned caseload of individuals with IDD. This includes maintaining monthly contacts, linking consumers to resources, and participating in the judicial system.
Requires a Bachelor's degree in Social Work, Psychology, or a related field, with a valid driver's license and vehicle. Experience working with IDD populations and bilingual skills are preferred.

This position is responsible for the provision of Service Coordination activities and assessments to assigned caseload. This includes, but not limited to, maintaining all required monthly contacts, linking resources of various providers to consumers, advocacy, and participating in the judicial system in accordance with the philosophy, goals and objectives of Texoma Community Center. Operates under trauma-informed and recovery-oriented models of care which align with CCBHC standards.
