Duties: Must be able to have onsite presence within the Twin Cities, Minnesota Attending bi-annual meetings with clients to monitor services Develop and maintain a collaborative relationship with clients serving as a role model for clients by demonstrating positive, proactive and respectful communications Assessing client needs and ensuring continuity of care. This may include outreach and consultation with other partners and providers.
