Locations
Perform comprehensive assessments for patients transitioning from rehab to home to prevent readmission. Document findings according to CMS regulations and communicate pertinent medical concerns to attending staff.
Requires an active Texas Nurse Practitioner license and board certification (AANP or ANCC) with a master's degree in nursing. Must have reliable transportation and the ability to work 10 hours per week.
1+ years of Nursing Home experience Experience in meeting the medical needs of patients with complex behavioral, social and/or functional needs Understanding of Geriatrics and Chronic Illness Understanding of Advanced Illness and end of life discussions Compensation for this specialty generally ranges from $90 - $155 per hour. Total cash compensation includes base pay and bonus and is based on several factors including but not limited to local labor markets, education, work experience and may increase over time based on productivity and performance in the role.
