Provides skilled nursing and high-tech infusion therapy services to clients in their homes on an intermittent basis. Responsibilities include developing care plans, performing clinical assessments, and supervising paraprofessional aides.
Must be a Registered Nurse licensed in the District of Columbia and a graduate of an accredited School of Nursing. Requires experience in acute care infusion therapy and current CPR certification.
Provides skilled nursing services on an intermittent basis to clients in their homes.
Assists in the development and implementation of the nursing care plan. Evaluates the effectiveness of nursing intervention and updates the client care plan as needed. Communicates with the physician regarding client's status as needed and per agency policy. Communicates with other team members in order to provide comprehensive client care. Assures that all OSHA standards are in compliance. Participates in committee activities and meetings. Discusses with the Director of Clinical Services on a weekly basis the client's condition and required care and frequency of visits to be scheduled for the following week. Assumes responsibility for self-development by continually striving to improve his/her nursing knowledge through educational programs, attendance at workshops and conferences, active participation in professional and related organization meetings and individual research and reading. Observes confidentiality and safeguards on all client-related information and intradepartmental data. Performs other duties as assigned by the Director of Clinical Services. Coordinates the Admission of the Client to the Agency Conducts initial and ongoing comprehensive assessments of the client's needs, both initially and as required. Works with Discharge Planners to identify and plan home infusion therapy referrals. Assesses the client on the initial visit, including an assessment of home safety, adequacy of refrigeration, storage and cleanliness. Initiates the client plan of care and makes necessary ongoing revisions and provides service in accordance with reimbursement, ethical, and legal guidelines. Initiates appropriate preventive nursing procedures. Administers venipuncture and handles insertion of intravenous needles and catheters. Prepares infusion solutions with the addition of medications. Initiates, monitors, and terminates infusion solutions and additives and establishes flow rates per physician orders. Prepares clinical and progress reports and makes referrals for other services as appropriate. Informs the physician and other appropriate personnel of changes in the client's needs. Trains both the client and caregiver and assesses the ability of client or caregiver to manage when staff is not present. Maintains and submits written clinical and Agency records as deemed necessary to include: an initial assessment, a care plan, daily notes, and weekly schedules, summaries of service, change orders, re-certification and discharges. Provides Skilled Nursing Care as Outlined in the Client Care Plan Nursing services, treatments and preventive procedures ordered by the client's physician that require substantial specialized skill. Initiates preventive and rehabilitative nursing procedures as appropriate for the client's care and safety. Observes signs and symptoms and reports to the physician the client's reactions to treatment, including changes in the client's physical or emotional condition. Teaches, supervises and counsels the client and family members regarding the nursing care needs and other related problems of the client at home. Supervises Aides Supervises and evaluates the care given by the paraprofessional per agency policy and submits completed written reports. Participates in periodic conferences with the Coordinator concerning the paraprofessional's performance. Job Conditions Travel is required, by car or airplane to local, out-of-town or state seminars, conferences or meetings. The ability to communicate well, both verbally and in writing is required. The ability to access clients' homes which may not be routinely wheelchair accessible is required. Hearing, eyesight and physical dexterity must be sufficient to perform a physical assessment of the client's condition and to perform client care. Physical activities include walking, sitting, stooping, and standing and lifting and turning of clients. Equipment Operation Use of multi-line telephone, calculator and medical nursing equipment, i.e., glucometer, etc. Confidentiality of Information Has access to all client medical records and client financial accounts which may be discussed with Director of Clinical Services, the Administrator, the Executive Director, the Director of Reimbursement, and the Quality Assurance Department.
Must be a graduate of an accredited School of Nursing. A BSN is preferred. Must be a Registered Nurse licensed by the District of Columbia. Must have experience as a professional nurse in an acute care setting performing infusion therapy. Must have a criminal background check. Must have current CPR certification.
