Provides personal hygiene care and companionship to hospice patients according to their individual Plan of Care. Responsibilities include monitoring patient conditions, delivering medical supplies, and reporting changes to the Nurse or Care Manager.
Requires a high school diploma or GED and a valid State Certification as a Certified Nurse Aide. Candidates must have at least one year of direct patient care experience and demonstrate written competency.
