Schedule Full-time, 40 hours per week Monday–Friday, 8:00 AM–6:00 PM (hours vary) Occasional weekend coverage for events (Saturday–Sunday, 8:00 AM–6:00 PM; hours vary) Position will never exceed more than 40 hours a week Mileage reimbursement provided for event-related travel Overview Works in collaboration with the Case Manager and other healthcare team members to provide comprehensive and coordinated care. Engages in interventions to improve self-management skills and improve quality of life for patients with chronic and/or complex illnesses. Provides education and behavior modification strategies and provides social support to patients and families. Acts as a patient advocate and healthcare navigator in assisting with resource allocation and management. This role works with a variety of grant funded programs to bridge the gap between organizations, including health care delivery systems and community health services to: increase culturally relevant knowledge about health and wellbeing; improve underserved populations’ access to services; connecting patients to community resources; and ultimately improving health outcomes.
