• Supportive, collaborative interdisciplinary team environment What You’ll Do As a Home Health RN, you will serve as the clinical leader for your assigned patients, coordinating and delivering skilled nursing care in the home setting. Key responsibilities include:
• Conducting comprehensive nursing assessments and developing individualized Plans of Care
• Managing a caseload of patients from admission through discharge
• Performing start-of-care visits, routine visits, recertifications, and discharges
• Evaluating physical, psychosocial, environmental, and functional patient needs
• Collaborating with physicians, caregivers, and interdisciplinary team members to ensure continuity of care
• Monitoring patient progress and adjusting care plans as needed
• Educating patients and families on disease management, medications, and self-care strategies
• Identifying early changes in condition and intervening to prevent avoidable hospitalizations
• Documenting all clinical findings, interventions, and outcomes accurately and timely
