Registered Nurse (RN) - Patient Navigator

This position may qualify for a sign-on bonus.

• Under general administrative direction, coordinates referrals from physicians and health care facilities for patients; provides a significant level of patient education related to their illness and planned treatment.
• Responsible for guiding the patients with orthopedic and cardiac DRGs through the complex care continuum.
• Assures patients receive care that consistently follows evidence-based standards and is in accordance with regulatory standards as defined by the regulatory agencies.
• Completes established competencies for the position within designated introductory period.
• Other related duties as assigned.
