ACT Team Nurse

• Assertive Community Treatment (ACT) Team Nurse performs complex services to assist individuals in gaining access to ACT medical services.
• The nature of work requires working with severely chronically mentally ill individuals through aggressive intervention, frequent in-person contact, crisis intervention, and long-term contact with individuals, families, and service providers.
• Major responsibilities include but are not limited to, monitoring individuals’ medication-related needs, working with an interdisciplinary team consisting of psychiatrists, community-based providers, and center staff to provide services outlined in the individual’s recovery plan, and providing coordination of individual’s needs as necessary.
• This nurse will serve clients in the community and may be required to drive a personal vehicle with reimbursement for mileage.
• Ability to communicate and work effectively in a fast-paced work setting; ability to plan, implement and prioritize assignments.
• Ability to document accurately.

Minimum qualifications

The position requires a current Texas Registered Nurse license. Requires current CPR certification. Must have a valid Texas driver’s license and acceptable driving record, as well as personal automobile liability insurance as required by the state of Texas.

Work Schedule: 8-5pm (Mon-Fri)

Salary $33.82/hourly

Preferred qualifications

Experience in providing community mental health services. Bilingual skills to include Spanish/English. Experience with ACT services preferred.
