The nurse identifies Medicare patients for Annual Wellness Visits and conducts initial and annual preventive care assessments. They collaborate with primary care providers to ensure follow-up care and connect patients with community resources.
Candidates must be a graduate of an approved nursing school with a current North Carolina or Compact RN license. A minimum of one year of nursing experience and current AHA BLS certification are required.
The Medicare Wellness nurse works in collaboration with the primary care provider and all members of the health care team to ensure the highest level of service is provided. Primary responsibilities include identification of Medicare patients appropriate for Annual Wellness Visits (AWV) and responsible for conducting the initial and annual preventive care visits. Assesses healthcare needs that require additional follow-up by primary care provider and assists patients in appropriate follow-up. Knowledgeable of community resources and assists patients in connecting with those resources. Responsible for implementing the philosophy, objectives, and standards of care of the Medicare annual wellness program. All duties and responsibilities must be carried out with empathy and compassion towards patients and their families, as well as coworkers.
