Provide advanced clinical support, symptom management, and face-to-face eligibility assessments for hospice patients across various settings. Collaborate with an interdisciplinary team to develop and implement goal-concordant care plans for patients and families.
Requires a BSN and a Master's or Doctoral degree in Nursing, along with a current NP license and DEA registration. A minimum of two years of experience as a Nurse Practitioner is required, with palliative care experience preferred.
Graduate of an accredited Bachelor of Science in Nursing program and an NP-focused Master’s or Doctoral Nursing Program Current Registered Nurse and Advanced Practice Nurse license in the state of practice Active prescriptive authority and DEA registration as required by state law Minimum of two (2) years of experience as a Nurse Practitioner; hospice or palliative care experience preferred Strong knowledge of palliative care, pharmacology, and end-of-life symptom management Effective communication and interpersonal skills with patients, families, and interdisciplinary team members Palliative care certification such as CHPN or ACHPN preferred but not required Reliable transportation, valid driver’s license, and current automobile insurance Commitment to delivering high-quality, person-centered care aligned with Arc Hospice & Palliative Care’s mission and values Arc Hospice & Palliative Care is an Equal Opportunity Employer and welcomes applicants from all backgrounds to join us in delivering compassionate, inclusive care.
