Provide emotional support, counseling, and psychosocial assessments for hospice patients and their families. Coordinate community resources and guide families through advance care planning and bereavement support.
Requires a Bachelor's degree in Social Work (Master's preferred) and state licensure. Candidates must have at least 2 years of social service experience in a healthcare setting and a valid driver's license.
What You Bring Bachelor’s degree in Social Work, Master’s degree preferred. Master’s degree from school accredited by the Council on Social Work Education required in states as specified. Licensure and/or certification as required by the State. Minimum of 2+ years social service experience in a health care setting or social services agency. Experience in hospice, palliative care, or a related healthcare setting preferred Deep comfort with grief, loss, and end-of-life conversations Strong knowledge of community resources and care coordination Excellent documentation skills and attention to compliance Self-directed with the ability to work with little supervision Valid driver's license and willingness to travel locally for patient visits
