Job Summary: The CIS Family Mobile Screener serves as a critical member of the Housing Services’ Coordinated Entry team, working directly with individuals and families experiencing homelessness to ensure timely access to housing as well as supportive resources. This position consists of field-based engagement, data management, and direct client navigation to promote a seamless, person-centered pathway from homelessness to stability. Job Duties: Maintain participant caseloads actively going through Coordinated Entry to assist with obtaining vital documents, completing housing referrals, and navigating barriers to placement. Act as a bridge of support through motivational interviewing, service connection, and advocacy for consumers experiencing housing instability. Complete service connections with consumers for improving mental and physical health, as well as other identified goals. Provide logistical support to participants, including transportation to essential appointments and assistance completing forms or applications. Maintain consistent communication with clients, ensuring they understand next steps and have access to community resources. Collaborate with outreach teams, shelters, and housing providers to complete referrals and ensure successful program placement. Complete outreach at places where people experiencing homelessness congregate. Enter, update, and maintain participant data within HMIS in accordance with privacy, security, and data quality standards. Identify service gaps, report system challenges, and recommend process improvements to enhance efficiency and equity. Perform other duties as assigned. Minimum Requirements: Current resident of the State of Wisconsin. Current Wisconsin Driver's License and access to a reliable insured vehicle; both maintained throughout employment. Must possess a high school diploma and/or GED equivalent. At least five (5) years of experience working with the homelessness population, supportive housing, and/or Human Services. At least Intermediate knowledge of all computer and software applications such as Microsoft Word and Microsoft Excel. Must be able to travel extensively, up to 70% of work time, within the community. Knowledge, Skills, and Abilities: Knowledge of housing programs and HUD regulations as they relate to homelessness and income guidelines; knowledge of available housing resources; knowledge of the permanent supportive housing model; ability to conduct clinical assessments to provide for proper discharge planning; ability to work with individuals with a wide range of needs; ability to effectively communicate; ability to read, write, instructions, reports, short correspondence and memos.
