Provide direct personal care to patients, including hygiene, feeding, and mobility assistance, according to a Plan of Care. Monitor patient conditions and report significant changes to the assigned RN Case Manager.
Candidates must hold a valid CA CHHA certification, BLS/CPR certification, and a valid CA Driver's License. Experience in home health care and working with high acuity patients is highly preferred.

All opportunities at 21HHS require being in the field visiting patients in their homes. Remote/work-from-home positions are not available unless specifically noted.
Provide personal care including, baths, back rubs, oral hygiene, shampoos, and changing bed linen Assist patients with dressing and undressing Provide skin care to prevent breakdown Assist the patient with toileting activities Keep patient’s living area clean and orderly, as appropriate Assist in feeding the patient, if necessary. Perform range of motion and other simple procedures as an extensional therapy service as ordered (with appropriate completed/demonstrated skills competency). Assist patient in the self-administration of medication. Meet safety needs of patients and using equipment safely and properly (foot stools, side rails, etc.). Report on patient’s condition and significant changes to the assigned nurse.

To be a success as a Certified Home Health Aide Valid CHHA CA Certification Current BLS/CPR Certification Valid CA Driver's License Excel at building rapport with patients and providing the highest quality of care Experience home health care is highly preferred Experience with high acuity patients is a plus Experience working with an interdisciplinary team is highly desired Self-Directed Empathetic Critical thinker Outstanding interpersonal skills Organized Prior to scheduling an in-person interview, all Candidates are required to complete a computer literacy and competency assessment. This is to ensure you are set up for success and can navigate in our environment including electronic charting, shared understanding of company values and use of our web based communication application.
