What you'll do
Deliver comprehensive primary care to CareMore ISNP members with complex chronic conditions in SNF, LTC, and institutional settings.
Conduct admission evaluations, routine follow-ups, and acute visits.
Develop individualized care plans including chronic disease management and advance care planning.
Coordinate transitions of care: hospital discharges, readmission prevention, medication reconciliation.
Collaborate with nurses, care managers, social workers, specialists, and facility staff.
Support STARs/HEDIS quality outcomes and reduce avoidable ER visits and readmissions.

The anticipated base salary range represents the Company's good-faith estimate of the compensation it reasonably expects to pay for this position at the time of posting. Actual compensation will be determined based on factors including experience, skills, qualifications, geographic location, internal equity, and business needs.
Candidates must hold an MD or DO from an accredited medical school and have completed a residency in Family Medicine, Internal Medicine, or Geriatrics. Board certification or eligibility in Family Medicine or Internal Medicine is required, along with a current medical license.
