The Case Manager develops and implements patient care plans while coordinating the continuum of care for individuals in an opioid treatment program. They are responsible for collecting clinical data and communicating medical criteria between insurance providers and the treatment team.
A Bachelor's degree in Social Work, Counseling, or a related Human Service field is required. Candidates should have 1-2 years of experience in a behavioral health setting, with a preference for experience in case management or utilization review.
