The RN Care Manager is responsible for delivering high-quality in-home nursing assessments and overseeing individualized pediatric care plans. They must monitor client conditions, conduct supervisory visits for home care staff, and collaborate with interdisciplinary care teams.
Candidates must have at least 2 years of recent RN experience in pediatric home care or acute care and a valid New York State RN license. Required certifications and documentation include BLS, current physical/PPD exams, and a valid driver's license with reliable transportation.

RN Care Manager Responsibilities Deliver high-quality in-home nursing assessments and case management services Develop, implement, and oversee individualized pediatric care plans Obtain and maintain physician orders in compliance with NYS regulations Monitor, document, and communicate changes in client condition Conduct supervisory visits and support home care staff in the field Ensure continuity of care while promoting client comfort, safety, and independence Collaborate with families, physicians, and interdisciplinary care teams
