• Friday: 7PM - 7AM Advanced Care Partners is seeking a Licensed Practical Nurse (LPN) to provide one-on-one care for medically fragile children and adults in home and school settings. This role offers the opportunity to deliver high-quality, individualized care in a more focused and meaningful environment.

• Pay: $35/hour Opportunity to earn up to $43/hour with applicable differentials:
• Trach: +$2/hour
• Vent: +$3/hour Nights (7pm–7am): +$2/hour
• Weekends: +$3/hour

Key Responsibilities Deliver skilled nursing care, including G-tubes, tracheostomies, ventilators, and central lines Monitor patient condition and respond appropriately to changes Educate patients and families on care plans and procedures Collaborate with interdisciplinary team members to ensure optimal outcomes QualificationsQualifications Active LPN license in Georgia or Nurse Licensure Compact A minimum of one year of LPN experience required. Current Basic Life Support (BLS) certification through the American Heart Association (AHA) or an equivalent accredited organization required.
