Job Summary

Provides direct patient care under the supervision of a Registered Nurse or other healthcare professional, following the established plan of care and treatment goals.

Status: Full-Time

Monday - Friday

Responsibilities

• Observes and promptly reports changes in patient condition or status to the RN Case Manager, Clinical Supervisor, or other appropriate healthcare team members.
• Accurately documents patient observations, measurements, findings, and care outcomes in accordance with established documentation standards.
• Monitors and records vital signs, including blood pressure, pulse, temperature, respiration, weight, and pain levels.
• Implements individualized care plans and documents patient progress and updates in a timely manner.
• Assists patients with activities of daily living, including bathing, personal hygiene, mobility, transfers, ambulation, and prescribed exercise programs.
• Performs supportive household tasks that promote patient independence, enhance self-care, and help prevent or delay the need for higher levels of care.
• Supports the onboarding and training of new staff by providing guidance on patient care practices, documentation requirements, and effective time management.

Education/Experience/Skills/Qualifications

• Current Home Health Aide certificate from the State of California
• Current CPR certification
• 1 year Home Health Aide experience preferred
• Bilingual in Spanish helpful

• Compensation is influenced by labor market conditions and various factors, including but not limited to: prevailing minimum wage in the area, relevant education, experience, skills, geographic location, performance, travel requirements, sales or revenue targets, and business needs.
