Deliver compassionate end-of-life and hospice care to patients in their homes under the guidance of an RN. Collaborate with interdisciplinary teams to implement care plans and manage patient symptoms and pain.
Must be a graduate of an accredited practical nurse program and hold a valid LPN license in the state of employment. Experience in hospice, palliative, or community health care is preferred.
Deliver nursing care in accordance with the State Nurse Practice Act, with a focus on end-of-life care. Implement current nursing practices based on comprehensive assessments and care plans. Maintain accurate and timely documentation of patient services. Provide patient and family/caregiver education and support. Recognize and address changes in a patient's condition. Collaborate with interdisciplinary teams, such as RNs, hospice aides, social workers, and more. Participate in Hospice IDT meetings to ensure coordinated care. Use and maintain equipment and supplies effectively. Manage pain and symptoms, evaluating patient responses to treatments. QualificationsQualifications: Graduate of an accredited practical nurse program. LPN licensed in the state of employment. Preferably, experience in hospice, palliative or community health home care or medical/surgical settings. Background in clinical auditing and chart reviews is a plus. Wound care experience is preferred. Understanding of hospice philosophy and the needs of the terminally ill. Basic knowledge of iOs operating system
