Schedule

Full-Time 40 hours per week Monday-Friday 8

00 AM – 5:00 PM - No required oncall, no required holidays

Responsibilities

• Provide compassionate direct and indirect nursing care to homecare patients and families throughout the service area
• Deliver nursing services including assessments, symptom management, preventative care, education, and time-of-death care according to the plan of care
• Work collaboratively across homecare teams to provide daily visits in patient homes, long-term care facilities, and assisted living facilities
• Complete assigned visits per day or as determined by clinical leadership based on patient acuity
• Communicate appropriately and promptly with physicians and other clinicians to support patient and family needs
• Maintain accurate and timely clinical documentation and physician orders within the Electronic Medical Records (EMR) system

Qualifications

• Current North Carolina Registered Nurse (RN) license required
• Strong nursing assessment and critical thinking skills
• Excellent organizational and problem-solving abilities
• Exceptional customer service and communication skills
• Ability to foster a positive culture built on respect, compassion, and teamwork
• Experience with electronic medical records required; Axxess EMR experience preferred
