Coordinates continuity of care for pre- and post-renal transplant patients to ensure efficient support for staff and healthcare professionals. Responsibilities include assessing patient status, implementing nursing interventions, and communicating critical data to physicians.
Requires a BSN or an ASN pursuing a BSN, along with a current Florida RN license. Candidates must have at least three years of nursing experience, including two years of medical/surgical experience with transplant populations.
The incumbent in this position coordinates the continuity of patient care for transplant patients in order to maintain organized and efficient support to staff and other health care professionals. Must demonstrate ability to assess data reflective of the patient's status and interpret the appropriate information needed to identify each patients requirements relative to age-specific needs and organ(s) transplanted and provide care according to protocols and with practice of nursing.
Identifying the patient's alterations in health; Assessing the patient's response to treatment protocols; Communicating appropriate data to physician staff; Implementing nursing interventions based upon established treatment protocols; and anticipating potential outcomes for patients at risk for complications.
