The Outreach Case Manager provides engagement, screening, and care coordination for unhoused individuals to connect them with behavioral health and substance use treatment. They serve as an on-site resource at the day center to reduce barriers to treatment and coordinate with community partners.
Minimum requirements include a high school diploma or GED and at least one year of behavioral health case management experience. A bachelor's degree in a related field and two years of experience are preferred.
The Outreach Case Manager role helps expand access to mental health and substance use treatment services for individuals experiencing homelessness. The Outreach Case Manager will provide engagement, screening, care coordination, and linkage to ongoing treatment services through Claratel Behavioral Health and other community-based resources.
Conduct outreach and engagement with day shelter guests who may have behavioral health and/or substance use treatment needs Provide brief screening for mental health and substance concerns Facilities referrals and linkages to Claratel Behavioral outpatient services; including counseling, psychiatric services, and substance use treatment Assist individuals with intake scheduling, transportation coordination, and follow up engagement to support treatment initiations and retention Coordinate with Day Shelter Staff to identify individuals who may benefit from behavioral health support Provide crisis-informed support and coordination when needed Assist with referrals and coordination with the DeKalb Regional Crisis Center when guests require urgent crisis stabilization Maintain documentation consistent with Claratel Behavioral Health policies and applicable regulations Other duties as directed by Program Manager and/or Director
High school diploma or GED At least one year of experience in behavioral health setting providing case management/community support services Must possess basic computer and typing skills. Must have care coordination skills to assist individuals with accessing/linkage to additional resources and services within the agency and/or community Must have transportation to and from work
Bachelor’s degree in related fields (such as human services, counseling, criminal justice, or social work) Two years of experience in a behavioral health setting providing case management/community support services NEEDED ATTRIBUTES Excellent interpersonal, problem solving and critical thinking abilities. Must be able to remain calm in crisis/mental health situations and respond appropriately. Always maintain confidentiality. Professional and effective communication with Community Partners, Stakeholders, Staff, Clients’ families/significant others and the Public.
