To support our continued growth and expand awareness of our Adult Day Health services, YourADH is seeking a Registered Nurse who is passionate about community engagement, relationship building, and connecting individuals and families with programs that enhance independence, health, and quality of life.
Key Responsibilities Provide nursing assessments for each participant upon admission, change in status and discharge.
Coordinate participant’s care with all relevant parties (family, guardian, physicians, nursing facilities, and residential providers) and maintain appropriate documentation of such care.
Supervise medication management services, nursing services, and medical treatment required for a participant’s inclusion in the program.
Maintain a database of medical documentation appropriate to a participant’s inclusion in the program.
Write and update nursing notes as necessary to address significant changes and health care teaching in each participant's health record.
Respond appropriately to any acute changes in a participant's health or daily activities.
Provide appropriate health care education for each participant and legally authorized representative.
Obtain reports and approval of treatment plans from primary care physicians, coordinate participant's care with the primary care provider and, if applicable, community service providers.
Review the nursing and medical services, developmental skills training program and needs of each member monthly and participate in the quarterly review process for each participant.
Ensure ongoing education, training, and professional development of all program staff that is relevant to care of participant.
Observe and monitor each client's participation within the program and document pertinent information in the progress notes including reasons for being absent from the program and follow up.

Monitor participant's individual dietary and supplemental feeding plan as prescribed by that participant’s own physician.

What We Are Looking For: RN Massachusetts license required Previous experience in an Adult Day Health setting or comparable work experience 1-2 years of direct service experience working with the elderly, chronically disabled, individuals with memory loss and/or Dementia Excellent knowledge of clinical standards and regulations Fluency in both English and Cape Verdean Creole or secondary language is preferred Strong time management and problem-solving skills with the ability to prioritize Outstanding written and verbal communication skills, with close attention to detail Strong organizational skills with the ability to multitask Able to work independently and in a team environment Physical and Environmental Demands Works in a clean well-lit environment with fluctuating temperatures in close proximity to others.
Requires substantial periods of repetitive work utilizing a computer, monitor, keyboard, and mouse.
Requires lifting and carrying equipment and supplies weighing up to 35 pounds; requires pushing and pulling equipment and supplies weighing up to 35 pounds; requires walking and standing; requires frequent sitting more than 75% of the workday; requires the ability to negotiate stairs; requires visual acuity and manual dexterity to operate equipment.
