Collaborates with a multidisciplinary care team to coordinate outpatient services and ensure appropriate levels of care for patients. Responsible for gathering clinical data, utilizing patient registries, and providing direct intervention including crisis de-escalation.
Requires a Bachelor's degree in psychology, social work, or a related field. Candidates must have at least 2 years of recent relevant experience and strong communication and organizational skills.
EDUCATION: Bachelor's: BA or BS in psychology, social work, or a related field. TYPICAL EXPERIENCE: 2 years' recent relevant experience. SKILLS AND KNOWLEDGE: Oral and written communication skills. Interpersonal and time management skills Ability to work effectively and prioritize tasks in a fast-paced environment with rapidly shifting priorities and competing demands. Ability to work independently with a minimum of direction. Intermediate PC skills and word processing skills required. This position will be on-site in both Santa Clara and Mountain View pediatric clinics. There is a possibility of a remote workday depending on space limitations, TBD.
