Physical therapist – home visits

About always best care

Always Best Care is a growing Medicare-certified Home Health agency helping patients throughout O‘ahu improve mobility, strength, balance, and safety while remaining in the comfort of their homes.

We are currently seeking a Physical Therapist to join our clinical team.

Pay: $80 per visit

Location: O‘ahu, HI (reliable transportation required)

Schedule: Flexible / Per Diem (PRN)

The opportunity

This position offers the ability to build a flexible caseload while providing one-on-one care in the home setting. You will work closely with patients, caregivers, physicians, and healthcare professionals to support positive outcomes.

Key responsibilities
Conduct evaluations and reassessments
Develop patient-centered treatment plans
Address mobility and balance concerns
Implement strength and fall prevention programs
Educate patients and caregivers
Maintain Medicare-compliant documentation

Minimum Qualifications

Current Hawaii Physical Therapist license
One year of clinical experience preferred
Strong communication skills
Reliable transportation
