About the role

Provide direct care and support to elderly or disabled participants, including assistance with daily living activities and health monitoring. Support the day center's operations through activity facilitation, environmental cleaning, and adherence to safety protocols.

Job Purpose

We are seeking a compassionate and dedicated Day Center Aide to join our team at CSC PACE Center. This role will provide support to elderly or disabled individuals, ensuring their comfort and well-being in a safe and caring environment. You will be responsible for assisting with daily activities, personal hygiene, and providing companionship to enhance the quality of life for the clients we serve.

Duties and Responsibilities

  • Participant Care Responsibilities
  • Assist participants with activities of daily living (ADLs), including:
  • Ambulation and transfers
  • Toileting and incontinence care
  • Grooming and personal hygiene
  • Feeding and hydration assistance
  • Dressing assistance as needed
  • Obtain and document vital signs, weights, intake/output, and other observations as directed by licensed nursing staff.
  • Observe and promptly report changes in participant condition, behavior, or safety concerns to the nurse or interdisciplinary team.
  • Assist with restorative care, mobility exercises, and fall prevention measures under the direction of nursing staff.
  • Maintain participant dignity, privacy, and confidentiality at all times.
  • Support safe participant transportation within the center and assist with wheelchair safety and positioning.
  • Day Center and Activity Support
  • Assist the Activities Department with setup, facilitation, supervision, and cleanup of group and individual activities.
  • Encourage participant participation, engagement, and socialization during activities and meals.
  • Escort participants to meals, activities, appointments, and rest areas as needed.
  • Provide supervision and redirection to participants to promote a safe and supportive environment.
  • Assist with meal service, snack distribution, and monitoring participants during dining.
  • Cleaning and Environmental Responsibilities
  • Clean and sanitize tables, chairs, activity areas, equipment, and commonly touched surfaces in accordance with infection control policies.
  • Assist with maintaining cleanliness and organization of the day center, participant areas, nourishment stations, and activity rooms.
  • Properly dispose of trash, soiled linens, and biohazard materials according to facility procedures.
  • Restock participant care supplies, PPE, linens, and activity materials as needed.
  • Report environmental safety hazards, maintenance concerns, or supply shortages promptly.
  • Compliance and Safety Responsibilities
  • Follow all infection prevention and control policies, OSHA requirements, and safety procedures.
  • Maintain active CNA certification and required trainings.
  • Adhere to participant rights, HIPAA confidentiality requirements, and PACE program policies.
  • Participate in staff meetings, trainings, competency evaluations, and emergency preparedness activities.
  • Support the interdisciplinary team in delivering quality participant care and services.
  • Other duties as assigned.

Qualifications

Education

  • High school diploma or equivalency preferred.
  • Current Certified Nursing Assistant (CNA) license/certification in the State of California.

Experience

• A minimum total of two years of working experience with frail or elderly population preferred.

Skills and Knowledge

  • Effective written and oral communication skills.
  • Demonstrated ability to problem solve and apply good judgment.
  • Ability to function well independently.
  • Ability to use or be trained in computer skills for the workplace.
  • Demonstrates necessary skills and knowledge as outlined in attached position-specific Competency Assessment Profile.
  • Able to speak Cantonese/Mandarin/Vietnamese strongly preferred

Other

Requires physical strength to perform the job.

Routine travel between PACE sites and in the community required.

Requires valid driver’s license.

Physical Demands

Must be able to remain in a stationary position 50% of the time.

Ability to occasionally move about inside the office to access file cabinets, office machinery, etc.

Able to operate a computer and other office productivity machinery, such as a calculator, copy machine, and computer printer.

Able to constantly position yourself to maintain files in file cabinets such as reaching with hands and arms, kneeling, crouching, etc.

The ability to communicate, detect, converse with, discern, convey, express oneself, and exchange information is crucial for this role.

Requires a current California CNA license and a high school diploma or equivalent. Preference is given to candidates with at least two years of experience with frail or elderly populations and multilingual capabilities in Cantonese, Mandarin, or Vietnamese.
