The Peer Health Navigator connects guests in residential crisis programs to community resources to support their transition back into the community. They manage discharge planning and provide follow-up navigation services for 90 days post-discharge to ensure stability.
Candidates must have a high school diploma and New York State Peer Certification (or obtain it within the first year). A bachelor's degree and bilingual skills are preferred, along with proficiency in basic computer software and data management platforms.

Our programs are spread across 25 locations and include over 1200 units of supportive housing in three boroughs, the Howie the Harp Advocacy Center, Peer-driven Crisis Respite Center, Assertive Community Treatment (ACT) and the Intensive Mobile Team (IMT).
The Peer Health Navigator’s primary focus is connecting Respite or ICR guests to community resources that can help support them as they move through the crisis and return to their lives in the community. This is a key part of successful discharge planning as it will help guests forge and sustain connections to a variety of professional and community-based supports. These peer navigation services would continue for 90 days after discharge to ensure successful crisis resolution and stability upon their return to the community.
