Lead holistic health and wellness initiatives for faith-based communities in North and Northwest Tulsa as a clinic-embedded Patient Navigator. Coordinate with community health workers to provide screenings, education, and care navigation to promote health equity.
Requires a high school diploma with 2 years of experience, an Associate's or Bachelor's degree, or 4 years of applicable job-specific experience. Proficiency in Google Workspace and experience as a Community Health Worker or Health Navigator is preferred.

What additional preferences we're seeking Proficient computer skills, to include Google Workspace CHW-C or at least 2 years of experience as a Community Health Worker (CHW), Health Navigator or Promotora Familiarity with community organizations and resources available to patients Available to work most Sundays and occasional evenings for community events in support of the Bedrock Initiative
