Coordinate care for long-term care residents by maintaining proactive on-site engagement and managing care plans via EMR. Serve as the primary liaison between clinical providers, facility staff, residents, and their families.
Requires a clinical certification or licensure (such as CNA, PCT, MA, LPN, or RN) and at least one year of patient-facing experience in a long-term care setting. Must possess a valid driver's license, reliable transportation, and proficiency in Microsoft 365.
Legally authorized to work in the United States of America Active, unrestricted clinical certification or licensure, such as: Patient Care Technician (PCT) Certified Nursing Assistant (CNA) Medical Assistant (MA) / Certified Medical Assistant (CMA) Licensed Practical Nurse (LPN) Registered Nurse (RN) Or comparable credential Minimum 1 year of patient-facing experience in a long-term care, skilled nursing, or similar clinical setting Comfortable communicating proactively and professionally throughout the day via Microsoft Teams, email, phone, and in person Cultivate and maintain positive, approachable relationships with residents, families, providers, and facility staff, while demonstrating a consistent, dependable presence within the long-term care setting Well-developed organizational and time-management skills, paired with an optimistic approach and initiative-driven mindset Strong working knowledge of healthcare terminology and care coordination principles Proficiency with Microsoft 365, including Outlook, Word, and Excel Ability to work full-time, Monday through Friday, 8:30 a.m. to 5:00 p.m. EST, (or thereabouts, contingent on facility hours) in an on-site role, based in the Atlanta, Georgia area, with a primary focus on the Peachtree City vicinity.
